Re: Claim Form Submission
Due to recently established accounting procedures, we can no longer accept
faxed claim forms as submission for payment.
Beginning with the fall 2006 season, original signed claim forms must be
either mailed or hand delivered to the Section XI office. In addition,
signed extra compensation forms (a.k.a. pink slips), must be attached to
each claim form. We can no longer accept a partner’s submission as claim
for the submitting official. It is also recommended that each official make
a copy of their claim form and attachments for their records prior to

IMPORTANT For all REFEREES
All referees when officiating a tournament
should obtain a copy of the signed tournament form from the head referee so
that they can include with their final end of season claim forms.
Head referees should either bring copies of all signed tournament forms to a
meeting for distribution, or email or mail a copy to each referee on the
